Community Health Worker Integration in Kentucky's Community Pharmacies

KPERF is applying for a Health Resources and Services Administration Grant with a project titled Community Health Worker Integration in Community Pharmacies - Kentucky. This project would include training Pharmacy Technicians employed by community pharmacies in rural Kentucky as Certified Community Health Workers and coaching those technicians and their pharmacies on activities to educate, prevent, and manage chronic conditions, with a focus on heart disease, through collaboration with other local providers. 

Pharmacies chosen to participate will receive funds to assist in the implementation of these services, as allowable by the grant (currently budgeted at $13,000/year). We will also be working with participating pharmacies on finding non-grant avenues of reimbursement for these services. Education of pharmacy technicians on community health workers would be provided at no cost. Regular check-ins and reporting on grant-related outcomes would be required.
