Apply Today for the Kentucky Board of Pharmacy


KPhA is seeking nominees for two (2) positions on the Kentucky Board of Pharmacy beginning in January 2025. KPhA selects and submits to the Governor a list of five (5) pharmacists, each of whom has had at least five (5) years experience in the practice of pharmacy, is a resident of the state and is in good standing with the board.
 

Under Kentucky law, the Governor of the Commonwealth of Kentucky takes these names under advisement as he makes appointments to the Kentucky Board of Pharmacy. KPhA has developed a process which helps ensure that the submitted candidates meet the minimum qualifications set in statute.  

To apply, please complete the forms below and return them to KPhA by March 31, 2024. If you require the forms in a different format, please get in touch with KPhA. Once submitted, please note that the Commonwealth-mandated demographics form authorizes the Governor’s office to conduct a background check on the candidate.
