Apply Today for Kentucky Board of Pharmacy Appointments

KPhA is seeking nominees for two (2) positions on the Kentucky Board of Pharmacy to begin in January 2022. KPhA can submit five (5) names for the open Board position to the governor for selection.
 
Under Kentucky law, the Governor of the Commonwealth of Kentucky takes these names under advisement as he makes appointments to the Kentucky Board of Pharmacy. KPhA has developed a process which helps ensure that the submitted candidates meet the minimum qualifications set in statute.
 
To apply, please complete the forms linked below and return to KPhA by April 19, 2021. If you require the forms in a different format, please contact KPhA. Please note that the Commonwealth-mandated demographics form, once submitted, authorizes the Governor's office to conduct a background check on the candidate.
 
Serving on the Kentucky Board of Pharmacy is both a privilege and an honor – governing the profession of pharmacy as a whole with the protection of the public as the ultimate goal.
